UIC "W" Agreement

This form is to be iniatiated by the College of Medicine departmental HR contact when a faculty member is to be hired under a definite term ("W" Agreement).

By execution of this special written agreement, a faculty member waives all rights to all tenure provisions contained in Article X of the University of Illinois Statutes.
