College of Applied Health Sciences Intercollegiate Transfer (ICT) Application

Please submit this application to apply for intercollegiate transfer (ICT) admission to the College of Applied Health Sciences for the majors in Community Health; Interdisciplinary Health Sciences; Kinesiology; Recreation, Sport, and Tourism; and Speech and Hearing Science. Please review the deadlines and minimums for admission to majors in the College of Applied Health Sciences as an intercollegiate transfer applicant before submitting this application.

Applicants may submit one application per period. If you wish to withdraw a submitted application, please contact ahs-ict@illinois.edu to make this request. You may withdraw a denied application so that you may apply for a different major in the same application period.

The application does not allow you to save your answers, so please allow yourself adequate time to complete the application. The application requires a personal statement of less than 500 words outlining your interest in the major, your career goals, and any experiences you have that relate to the major. You are encouraged to write this statement before starting the application so that you may paste it into the form.
