Proctoring Services for Individuals Payment

This form should be used only if you have scheduled a testing appointment and completed registration with the Testing Center. Completion of this form indicates that client will accept the terms and conditions below and abide by Testing Center security protocols.

Disclaimers: The Testing Center proctors individuals taking exams to ensure academic integrity. If for any non-client related reason the exam is not deliverable the appointment will be rescheduled at no additional fee. Clients agree to adhere to security requirements as well as exam owner conditions. Payment of fees quoted at the time of the appointment are collected the day of the exam. If client does not complete the exam for their own reasons fees will not be refunded. Neither party shall be liable for damages if any delay or default is caused by conditions beyond control (i.e., force majeure).
